8D – 8 Disciplines
D0: Plan
Plan for problem solving – defining a problem, clarifying, setting goals, identifying causes, developing action plans, execution, evaluation, and continuously improving. Determine prerequisites.
D1: Form a Team
Establish a team of people with knowledge of the process or product.
D2: Define and Describe the Problem
Specify the problem by identifying the following: who, what, where, when, why, how, and how many.
D3: Take Interim Containment Actions
Determine, define, and implement containment actions to isolate the issue.
D4: Analyze the Root Cause
Identify applicable causes to explain why the problem occurred. Identify why the problem was not caught at the time it arose. Causes should be verified through the means of a detailed investigation.
D5: Formulate and Select Corrective Actions
Confirm that the selected corrective action will resolve the issue that originally occurred.
D6: Implement and Validate Corrective Actions
Define and implement the best corrective actions.
D7: Identify and Implement Preventative Actions
Modify the management systems, operation systems, practices, and procedures to prevent recurrence of the issue.
D8: Recognize the Team and Individuals
Recognize the collective efforts of the team! A great deal of effort and time is dedicated in addressing and resolving issues.
